Any uses not listed here require your explicit written authorization, which you can withdraw anytime by notifying us in writing.
A. Treatment – We may use and share your PHI to deliver prescription and supply services to you. We might disclose your PHI to other pharmacists, pharmacy technicians, and health care providers involved in your treatment. You will get separate notice and a chance to opt out of any subsidized treatment-related communications.
B. Payment – We will use and disclose your PHI to bill and receive payment for the services we provide. This may involve sharing your PHI to get pre-approval from your health plan or confirm coverage for a prescription or service.
C. Health Care Operations – We may use and disclose your PHI for managing our pharmacy operations, such as quality reviews and improvements, compliance audits, and staff performance assessments. We might also use your PHI for general business management and administrative purposes.
D. Prescription Refill Reminders, Treatment Alternatives or Health-Related Benefits – We may use and disclose your PHI to reach out about refill reminders, inform you of alternative treatments, or share details on health-related benefits or services that could interest you.
E. Family Members, Relatives or Close Friends – Unless you object, we may share your PHI—with limits to what's relevant—with family members, relatives, close friends, or others you identify as involved in your care or payment. If you're not available to agree or object, we may use professional judgment to decide if disclosure serves your best interest, sharing only pertinent PHI.
F. Other Permitted and Required Uses and Disclosures – We may use your PHI without your authorization or chance to agree/object in these cases
You have several rights concerning your PHI as our patient. Here's what they include:
A. You can request restrictions on our use/disclosure of your PHI, though we aren't required to agree (except for fully self-paid transactions). Submit written requests detailing: (1) the desired restriction; (2) the information affected; and (3) who it applies to (e.g., a spouse). If agreed, it won't block disclosures: (1) for your access/accounting requests; (2) as legally required/permitted; or (3) in emergencies.
B. You can request confidential PHI communications through alternate methods or locations (e.g., sending prescription info to a different address). Submit written requests to the Privacy Officer specifying your preferences. We will accommodate reasonable requests.
C. You can access, inspect, and copy your PHI (including electronic), except for certain HIPAA-exempted items. We'll provide requested formats for electronic PHI if maintained that way. If not in our possession, we'll direct you appropriately. Copies may involve reasonable cost-based fees for copying/postage, with timely responses. Limited denials allow review requests; ultimate denials include written explanations.
D. You can request an accounting of our PHI disclosures (including by business associates) for up to six years prior (or shorter if specified). One free annual request; subsequent ones in the same 12 months may incur reasonable fees (with prior notice and option to withdraw/modify).
E. If you think your PHI is inaccurate or incomplete, submit a written amendment request with supporting reasons. We may deny if, e.g., not created by us or already accurate. Denials allow your written disagreement statement; we may rebut, and relevant documents can be included in future disclosures.
F. You can request a paper copy of this Notice anytime, even if received electronically—submit written requests to the Privacy Officer.
G. You can opt out of fundraising; we won't use your PHI for fundraising or sell it without your prior authorization.

If you encounter any problems with your order or medication, please contact our support team right away.
You can terminate your subscription whenever you wish by accessing your secure, password-protected account dashboard. The rules below govern cancellations and refund eligibility specifically for prescription-based orders:
Cancellation Before the Medical Consultation: If you send a written cancellation request by email to support@xyrxhealth.com before your consultation with a licensed physician is finished, you may qualify for a refund (subject to approval). Approved refunds will be issued.
No Refunds or Cancellations After Consultation Completion: Once the consultation is done and a prescription is approved, it is sent right away to the partnered pharmacy for preparation and dispensing. From that moment, the order becomes final and cannot be canceled. No refunds are available, no matter the delivery stage. This follows strict U.S. FDA guidelines that ban the return or resale of any dispensed prescription drugs.
If the Prescription Is Not Approved: Should the physician decline to issue the prescription, you will get a complete refund with no deductions or fees applied.
Requests to Modify a Prescription After It Has Been Sent to the Pharmacy: If you want to switch to a different prescription after it has already gone to the pharmacy, a $150 re-processing charge will apply. Any changes require re-evaluation by a licensed doctor and are not always approved.
Compliance with Regulations: Prescription orders become irreversible once forwarded to the pharmacy. Per FDA rules, dispensed or prepared prescription medications cannot be returned or refunded.
Subscription Details: All charges for services and products are handled through automatic recurring monthly subscriptions. These renew each month until you cancel. You need to supply a current payment method during signup and keep enough funds available for ongoing charges. Cancel anytime via your protected dashboard.
Required Commitment for Weight Loss Programs: Programs involving medications like Tirzepatide or Semaglutide come with a mandatory three-month term, since three months' supply is provided in advance. You must complete this full period regardless of your payment schedule. Canceling early triggers an early termination fee equal to the standard monthly retail rate for the program.
Monthly Installment Options: These are available as no-interest plans without credit checks. If you cancel a monthly plan before finishing the three-month obligation, the early cancellation fee still applies. It is your responsibility to make sure all scheduled payments go through successfully.
Chargebacks: Attempting to reverse or dispute a valid charge (a "chargeback") for delivered services or fulfilled orders counts as a violation of these terms. We will actively challenge any such disputes and supply evidence to the card issuer, such as records of the consultation, prescription approval, and order status. If a chargeback is filed without first trying to resolve the matter with us directly, you may face a $199 chargeback handling fee added to your balance. We also retain the right to take legal steps and seek reimbursement for related expenses, including attorney fees and court costs.
Declined Payments and Overdue Balances: Participants in three-month programs remain responsible for the entire commitment even on installment plans. If payments fail and the full amount isn't covered, the unpaid portion becomes delinquent. We may send overdue amounts to a collections agency, and you will then owe all collection-related fees plus a 50% surcharge on the remaining balance. Legal recovery options may also be pursued at your expense.
Medications (such as sterile GLP-1s and peptides) are shipped overnight in specialized cold packaging with insulation and ice packs to maintain quality. Products undergo lab testing for stability during transit and can safely handle room temperature (under 86°F/30°C) for up to 10 days or brief higher exposures—refrigerate promptly upon receipt.
